If any of the following apply, you may be eligible for free services through a Medication Therapy Management (MTM) program: 

You're enrolled in a Medicare prescription drug plan. 

You take drugs to treat a variety of ailments. 

This program assists you and your doctor in ensuring that your drugs are improving your health. A pharmacist will give you a comprehensive review of all your medications and talk with you about:

• How well your medications are working
• Whether your medications have side effects
• If there might be interactions between the drugs you're taking
• Whether your costs can be lowered
• Other problems you’re having 

You'll receive a written summary of the conversation, as well as an action plan outlining what you can do to get the most out of your drugs. 

There will be space for you to take notes or jot down any queries you may have. 

You'll also receive a personal medicine list, which will detail all of the medications you're taking as well as why you're taking them. 

It's a good idea to schedule your medication review before your annual wellness visit so you can discuss your action plan and medication list with your doctor. 

Bring your action plan and prescription list to your appointment, as well as any other conversations you have with your doctors, pharmacists, or other health-care providers. 

Also, remember to bring your medicine list.
